LTO Student Permit Application Requirements
FOR FILIPINO APPLICANTS
QUALIFICATIONS:
- Must be at least 16 years old.
- Must be physically and mentally fit to operate a motor vehicle.
- Must be able to read and write in Filipino or English.
- Must not be a drug user and/or alcoholic.
- Must be clean, neat and presentable.
REQUIREMENTS:
(For 16-18 Years Old)
A. Download, Print and Fill Up the LTO APPLICATION FOR DRIVER’S LICENSE (ADL) FORM.
B. Proof of Identity: (Any of the Following)
- Original and Photocopy of Birth Certificate.
- National Statistics Office (NSO) authenticated Birth Certificate (Certified True Copy) with Official Receipt.
C. Taxpayer’s Identification Number (TIN)
D. Notarized Parent / Guardian letter of consent.
(For 19 Years Old & Above)
A. Download, Print and Fill Up the LTO APPLICATION FOR DRIVER’S LICENSE (ADL) FORM.
B. Proof of Identity: (Any of the Following)
- Photocopy of Birth Certificate.
- Authenticated Birth Certificate with Official Receipt.
- Photocopy of Passport with Original Valid Passport
- Any legal or government issued documents. (SSS ID, PhilHealth ID, GSIS ID, Pag-Ibig ID)
C. Taxpayer’s Identification Number (TIN)
If the Birth Certificate is unavailable, any of the two can be used as replacement:
- Joint affidavit of two disinterested persons who know the applicant and who can attest to his/her age and identity accompanied by non-availability of record from NSO or local civil registry office.
- Any legal document to prove his age and identity (GSIS, SSS, Passport or any Government Issued ID.)
FOR FOREIGN APPLICANTS
QUALIFICATIONS:
- Must be at least 16 years old.
- Must be physically and mentally fit to operate a motor vehicle.
- Must know how to read and write in Filipino or English.
- Must not be a drug user and/or alcoholic.
- Must be clean, neat and presentable.
REQUIREMENTS:
A. Download, Print and Fill Up the LTO APPLICATION FOR DRIVER’S LICENSE (ADL) FORM.
B. Original and Photocopy of Alien Certificate of Registration (ACR)
C. Original and Photocopy of valid Visa/Passport and any of the following:
- Has stayed in the country for at least one (1) month and permitted to stay in the country for at least five (5) months.
- Employment contract certified by an employer to stay in the Philippines for at least five (5) months.
D. Additional requirements for minors. (16-18 Years Old)
- Notarized Parent / Guardian letter of consent.
- Student’s School ID.
